I selected 5 screens, but I'm only going to list 4 as I think some of the screens will need to broke into a detail screen, where the user can edit parts of the info.
"Background information" screen will ask for name, date of birth, social security number, home address, if they have been previously divorced (if so, dates), etc.
In the "Family Information > Children" screen will need to be way to add / remove / edit children. If the children are living outside the home (either because they are adult children or living with a previous spouse), we need their home address, phone number, e-mail address, etc.
